Where should you start when a QR code will not scan?
Separate file problems from print problems. Open the original SVG or PNG on a second screen and scan it with the phone that failed. Then try another current phone. This quick split tells you where to look.

Is the quiet zone missing or crowded?
A QR code needs blank space around all four sides. DENSO WAVE specifies a quiet zone four modules wide. A module is one square in the QR grid, so the required physical margin grows with the printed module size.
Cropping that border, placing text inside it, adding a decorative frame too close to the code, or letting a background pattern invade it can stop a reader from locating the symbol. Restore the complete border from the original export. Do not rebuild it by eyeballing a few white pixels.
Does the code have enough contrast?
Keep the modules darker than the background and avoid transparent or visually busy areas behind the symbol. The safest baseline is still dark modules on a light, solid field. Inverted, pastel, gradient, or photographic designs leave less room for weak lighting and mediocre cameras.
Screen colors do not guarantee printed colors. Ink spread, paper tone, laminate, and a printer's color conversion can close a contrast gap that looked generous on a monitor. Judge the final proof, not the picker swatches.
Is the QR code too small or too dense?
A camera reads modules, not the marketing team's intended dimensions. DENSO WAVE documents 40 QR versions, from 21 by 21 modules to 177 by 177. More encoded data generally selects a higher version with more modules. Printing two codes at the same width can therefore produce very different module sizes.
Increase the physical code size when the URL is long, the viewer is farther away, the surface is rough, or the print process softens edges. There is no honest universal minimum for every payload and use case. The useful minimum is the smallest size that passes repeated tests in the final conditions with breathing room to spare.

Was the file stretched, blurred, or recompressed?
A QR code grid must stay square. Stretching only its width or height turns square modules into rectangles. Screenshots, low-resolution exports, image filters, aggressive compression, and soft resizing can blur the hard boundaries a decoder needs.
Return to the original download. Use SVG when the layout accepts vector artwork, or place a sufficiently large PNG without enlarging a tiny copy. Do not copy the preview from a browser, chat app, slide, or social feed. Those paths love “helpful” image processing.
Is the final surface or lighting interfering?
A perfect file can fail on a reflective bottle, curved cup, wrinkled label, textured fabric, folded menu, or dim wall. GS1's 2D barcode printing playbook calls out substrate, glare, deformation, printing consistency, and final verification as practical scan factors.
- Move the code away from folds, seams, edges, and tight curves
- Use a matte patch beneath it when glare hides modules
- Increase size when texture or absorbent stock softens edges
- Add light or change placement when the camera cannot focus
- Replace scratched, faded, stained, or torn copies
- Test at the real viewing angle and distance
Is a logo or decoration covering functional data?
Logos work by hiding part of the QR data and relying on error correction. Higher correction levels add redundancy, but DENSO WAVE warns that decorated or deformed codes may become slow or impossible to read. A 30% Level H restoration figure is not a safe 30% logo-size allowance.
Remove the logo and retest. If the plain code works, make the logo smaller, keep it centered, preserve the three large finder patterns, use a clean background plate, and test again. Every styling choice spends some of the reliability budget.
Does it scan but open the wrong thing?
Read the decoded value before opening it. Confirm the scheme, domain, path, query string, capitalization, and punctuation. ZXing recommends including the full protocol so readers recognize a URL reliably.
If the decoded address is correct but the page fails, open that same address manually. Check the domain, page, HTTPS certificate, network, and any redirect service.
